We are reaching out to patients who were hospitalized at UNC and are also eligible for cardiac rehabilitation. We want to learn more about what the barriers are to attending cardiac rehabilitation. The purpose of this project is to identify and target barriers faced by rural endometrial cancer patients in order to develop a patient navigation intervention. Rural residents with endometrial cancer, the the most prevalent gynecologic cancer in the US, have higher odds of mortality than urban endometrial cancer patients. As the number of new endometrial cancer cases is expected to increase 50% by 2030, this urban-rural disparity is expected to continue widening.
Aim 1: Deploy and evaluate our implementation facilitation program and toolkit on implementation success of infusion clinics in nine sites using the RE-AIM framework. Aim 2: Evaluate the impact of implementation of infusion clinics on secondary patient outcomes including time to first dose of parenteral pain medications, rate of hospitalizations, and patient satisfaction with pain management.
