What will be asked of you

The registry will ask about the food(s) to which you are allergic. The registry also asks about eczema, hay fever, or asthma. A secondary survey asking details about your specific food allergy reactions will follow for you to fill out after completion of the initial consent and introduction survey. The first survey takes about 10-15 minutes. The second survey may take as little as 5 minutes or as much as 30 minutes, depending on how many food allergens you select. You may opt to provide an additional blood tube if your allergist is already planning to test your blood during your routine allergy visit.

Total length of participation:
Once only or once a year for the duration of the study
