Hospital to Home Care Coordination for Children and Youth with Special Health Care Needs

The purpose of this study is to compare two different ways of supporting children and families as they return home from a hospital stay. During the weeks following a hospital stay, children and families face many challenges. The results of this study will help us learn how best to support their needs during this vulnerable time.
